Anal Cancer
Screening

Anal cancer usually begins as anal intraepithelial neoplasia, or AIN, a precancerous change that can be found and treated years before it becomes cancer. Dr. Kratz's screening program follows the protocol proven by the landmark ANCHOR Study.

Who should be screened

  • HIV-positive individuals
  • Men who have sex with men
  • Patients with an abnormal anal Pap test
  • Individuals with a history of HPV infection
  • Patients with cervical, vulvar, or vaginal dysplasia
  • Immunosuppressed patients
  • Patients with persistent anal lesions

Screening guidelines are based on risk factors, not symptoms. Anal cancer rates have risen in recent decades, particularly among patients with HPV or immune suppression, and most cases are preventable when caught early.

What it actually feels like

  • The area is numbed with topical and local anesthesia before anything begins.
  • Nitrous oxide is available if you want additional comfort during the procedure itself.
  • Most patients describe pressure rather than pain, with manageable soreness for a day or two afterward.
  • No sedation is used, so you will never need a driver.

Built on the ANCHOR Study

The ANCHOR Study demonstrated that treating high-grade anal dysplasia significantly reduces the risk of developing anal cancer. It fundamentally changed how anal cancer prevention is practiced, and it's the standard this program follows.

  • Targeted biopsy of any abnormal tissue
  • Ablation of precancerous lesions
  • Ongoing surveillance for recurrence

Your visit

  • Screening performed through high-resolution anoscopy, using magnification to evaluate the anal canal
  • Biopsy of any concerning areas, if needed
  • A clear explanation of your results before you leave

Most screening exams take 20 to 30 minutes and are performed in the office.
